Observing transfusionists - how often?

For entities that do transfusions - do you have a flat count or a percentage of transfusions that you observe for compliance? What is it?

We are going to increase ours, as it is a very small number that we audit. Just looking for some standards in other hospital settings.

I worked at a 750 bed hospital and we would audit 2 transfusions per floor/unit. I am currently working at a significantly smaller hospital (177 bed) and we are working on an initiative to improve our transfusion practices. Part of this includes an attempt to audit all transfusions, but in actuality it ends up being about 50% of transfusions. After the initiative is over, I would say that we will be auditing at 5% of transfusions.
