Hello all, I work on the industry side of things, specifically in the ROSE sector. I recently heard that pulmonologists are trying to bill 88172 when using certain new biotech devices that don't necessarily render slides. Is this possible? Would it actually be reimbursed, since there is no pathologist involved? I am looking for someone who is an expert in medical reimbursements.

The code description includes the phrase when read by a cytopathologist or other "qualified" medical professional.  It has to be read on-site as the specimen is being collected.  If the individual is trained, this could be interpreted as being reimburseable.

    Hi Ricky,

I believe this code is for Cytology sample collection for onsite cytology analysis. According to code 88172 this would fall under the Pre-Analytic section and it should have reimbursement.

However, many organizations combined this kind of procedure as a bundle payment with all other patient services if it's outpatient or inpatient services.

For e.g. Bone Marrow sample collection procedures.

    Thank you,
    Nila

88172 is for immediate cytohistologic study of an FNA to determine the adequacy of the specimen for diagnosis.  It is billed per site and evaluation episode.   It is a physician billed code.  Then 88173 is for the final interpretation and report and is billed per episode.

    Yes, CPT code 88172 is

reimbursable, but reimbursement depends heavily on the specific clinical context and the individual payer's policies.

    Key Considerations for Reimbursement

    • Service Performed: CPT code
88172 describes the immediate cytohistologic study and interpretation of a fine needle aspirate (FNA) specimen to determine specimen adequacy while the patient is still present.
    • Components: Reimbursement can be split into professional (physician's interpretation, modifier 26) and technical components, which are billed separately depending on who provides the service and equipment.
    • Limitations: The code 88172 should not be reported for an interpretation by a medical laboratory scientist alone; it is for the physician's  or cytotechnologist's assessment of the specimen's adequacy.

To ensure reimbursement, providers should verify coverage policies with specific insurers and adhere strictly to all documentation and coding guidelines provided by sources like the Centers for Medicare & Medicaid Services (CMS) and the College of American Pathologists (CAP).

This reply sounds very much like it was at least partially written by AI - undisclosed use of AI is definitely not what we want to see on a forum such as this one!
